Applying Adolescent Health Equity to School and Community Healthcare: Community Outreach and Rally for Engaged Aggregate in Health Care (Co-Reach) Initiative

Purpose: This initiative enhances and improves adolescent health equity in an underserved community through identification, engagement and participation of residents (parent and child) who have little or no access to primary care by utilizing peer health navigators to provide early identification of health problems via screening and linkage to early and continuous treatment of health problems. The initiative is located within Liberty City which has high rates of teen truancy, obesity, HIV/ STDs, infant mortality, low birth weight, and a plethora of acute and chronic health conditions.
